Our online verification system will generate an affiliation letter as a primary source verification to reflect the appointment dates and specialty of a provider. You are responsible and liable for securing the appropriate consent and release. In addition, you will be required to attest to this prior to obtaining affiliation verification on this website. Applications

Physicians, Nurse Practitioners and Physician Assistants seeking credentialing may request an application for membership and clinical privileges on the medical or allied health staff at one or more of our hospitals. Simply email [email protected] with the following:

• NPI number
• Clinical specialty
• Office mailing address with phone and fax numbers
• The hospital(s) where you seek privileges
